The Director of Home Health is responsible for strict adherence to the minimum standards for clinical/clerical related services. The Director of Home Health is responsible for ensuring Home Health Standards are being met.Knowledge, Skills and Abilities RequiredJob RequirementsLead by and live the core values of our client - Safety, Passion, Integrity, Respect, Transparency and TrustDetail oriented and self-directedStrong organization and leadership skillsDemonstrated knowledge and skills necessary to communicate effectively with office staff, branch staff, and managementBroad-based knowledge of home health nursing and competent clinical skillsValid driver’s license and current auto liability insuranceAdequate means of transportationComputer, internet access and email capabilities from home or other off-site locationEducationRequired, a RN graduate of an approved school of professional nursingRequired, currently registered in the state of practiceTraining and ExperienceRequired, has five (5) years of supervisory and/or administrative experienceRequired, has three (3) years of providing direct patient care in the home settingRequired, seven (7) years’ experience as a registered nursePreferred, working knowledge of Medicare eligibility requirementsPreferred, experience of training employeesEssential Functions (to include but are not limited to)Promote and coordinate interdisciplinary activities to ensure that the needs of patients, staff and physicians are met in accordance with established standards for all offices.Supervise daily clinical functions that occur within the agencyProvide immediate support of incoming referrals or patient related telephone calls in the absence of assigned associateObtain daily report from field staff performing Oasis visits to ensure that our client’s programs are being utilized efficiently to meet individual patient needs, coordinate with physician for verbal order to implement POC, and scheduler so that ordered services are provided as planned.Direct implementation of approved work methods and procedures that reflect elements essential to rendering high quality careEnsure that the professional standards of nursing practice are maintained by all nurses providing careParticipate in the review, analysis, and appraisal of the effectiveness of the total agency programAssure compliance with local, state, and federal regulationsSupervise and teach in order to improve practice within the agencyPromote an environment in which the patient care team can work cooperatively toward achieving individual and agency objectives.Provide leadership in promoting and maintaining standards for giving good quality care by all members of the patient care teamEncourage and assist personnel to continue self-improvement through formal education, educational meetings, and active participation in professional and related organizationsAssist with the employee health program/safety infection controlAssist with risk management and trending incident reportsSelect and maintain a qualified, well organized staff to provide for the needs of the patients.Oversee the establishment of methods for coordination of care by all disciplines.Direct staff in planning, developing and evaluating the formal orientation and staff development programs provided for all nursing staffEstablish staffing patterns which reflect the quality and quantity of various professional personnel necessary to plan, provide, and supervise the rendered to patients and families. Participate in rotating administrative call schedule.Participate in Performance Improvement activities.Assist with case conference preparationAttend mandatory case conference meetings as directedAssist in acting as a liaison between agency and contracting agencies.Will demonstrate and exemplify all of our client’s core values in areas of work ethic, communication and most importantly, employee and patient relations.Working ConditionsWorks with computers on a daily basisTypically the employee will sit for extended periods of timeWalking, standing, reaching, bending, and carrying of light to heavy items such as papers, office/medical supplies, books and files on a regular and recurring basisMust be able to communicate articulatelyModerate amount of travel is required
